Effective Monday, June 26, 2017, Nasdaq Market Regulation and Nasdaq Market Operations will begin accepting opening obvious error and catastrophic error trade review requests for the for Nasdaq ISE, Nasdaq GEMX and Nasdaq MRX (collectively "Nasdaq ISE exchanges") electronically through the Nasdaq Obvious Error Online Submission form.

Member firms will be able to utilize the online submissions form for all six Nasdaq options markets to request obvious and catastrophic error trade reviews. Please use the link below to access the online form.

Nasdaq Obvious Error Online Submission Form

From June 19, 2017 through June 23, 2017, in preparation of the change to the handling of the request for obvious and catastrophic error reviews, Nasdaq Market Regulation requests that all Nasdaq member firms submit "TEST" submissions for the Obvious and Catastrophic Errors for the Nasdaq ISE exchanges to ensure that each authorized staff can submit a form from their designated PCs.

Please designate the Root Symbol and Error Details fields as "TEST". Please remember to use your firms' current phone number and e-mail address. You will receive a confirmation disclosure e-mail verifying the submission was successful. Nasdaq Market Regulation will confirm your submission with a "Submission Received" response from Exchange Officials. If you do not receive an email disclosure or get a blank screen after clicking on the submit button, you will need to upgrade your web-browser to the latest version of JAVA and/or Adobe Flash.

Effective Monday, June 26, 2017, Nasdaq ISE exchanges will no longer accept email requests for obvious or catastrophic error trade reviews.
